Please contact our U.S. headquarters in Smithfield, RI - 888-612-7774 for a materials authorization number prior to the shipment of product. The Materials Test Form can be downloaded and printed to be included with the shipment of material. Once received we will contact you with a test schedule date and provide you with the results. If you wish to attend testing please indicate so on the materials test form.
